Request an Appointment

Tell us how to reach you and which office you prefer.

Please don’t include medical records, insurance member numbers, or detailed health information. Our team will follow up with you. For a medical emergency, call 911.

Thank you. Your request has been sent. Our team will contact you; an appointment is confirmed only after we speak with you.

How to schedule an appointment online

Fill out the appointment request form on this page. Include your preferred office, the days and times that work for you, and a short reason for your visit, such as neck pain or a car accident. Please keep detailed medical information out of the form.

After you send it, our team will contact you to confirm a day and time. Your request isn’t a confirmed booking until our team confirms it with you. For a medical emergency, call 911.

What to have ready

Having these on hand when you schedule an appointment saves time:

  • Your insurance card
  • A referral or prescription, if you have one
  • For a car accident: the accident date, your claim number and the insurance company’s contact details
  • For a work injury: your employer’s name and your workers’ comp claim number, if you have one

New to PMR? Our new patient information explains what to bring and what happens at your first visit. You can also check your plan on our insurance and billing page.

Cancellations and changes

Please give at least 24 hours’ notice if you need to cancel or change an appointment. Missed appointments may be charged a no-show fee.
